矫正鞋垫可以治疗O形腿吗？

1月7日，据天津日报报道，天津市公安机关，已经依法对束某某等人进行刑事拘留。至此，沸沸扬扬的权健事件，总算告一段落。相关新闻（图片来源：微博）不过，卖保健鞋垫的不止权健一家，宣称能治疗O形腿的大有人在，鞋垫到底有没有医疗效果呢；另一方面，新手父母常常担心，我家孩子是不是O形腿啊、跟用纸尿裤有关系吗，甚至有人提出，可以捆绑婴儿的双腿、矫正腿型。捆绑“矫正”O形腿（图片来源：微信）要想回答这些问题，必须先问一句：什么是O形腿？O形腿就是我们常说的罗圈腿，与X形腿相对。其诊断方法也很简单：首先自然站立，接着脚跟与脚跟并到一起、脚掌与脚掌并到一起。在这个过程中，如果小腿还没并拢膝盖就撞到一起，便是X形腿，临床上称之为膝外翻；如果小腿并拢之后两膝盖间存在一定的距离，便是O形腿，临床上称之为膝内翻。[1]至于病情轻重，可以拍摄X片，根据胫骨与股骨的夹角进行评判；[2]也可以直接测量两膝盖之间的距离。自然站立时，两膝盖距离越远、说明O形腿的程度越重。一种评估膝内翻的方案（图片来源：Development of Tibiofemoral Angle in Korean Children）根据原因，可以将O形腿分为三类。一类是生理性的。因为子宫的体积有限，胎儿必须适度蜷缩，所以新生儿都是罗圈腿。随着生长发育，O形腿的情况会逐渐缓解；两岁半以后又反过来了，开始出现X形腿。[3]一般来说直到6岁，儿童的腿型才开始向成年人转变，等到10岁时，多数儿童的腿型便正常了。[4]儿童的腿型变化（图片来源：小儿骨科学）第二类是运动导致的，过早参加体育训练或者不当使用学步车，都可能引起O形腿。[5]尤其是后者，很多家长生怕小孩输在起跑线上，以为学步车可以帮助小孩学习走路，其实学步车不仅会干扰正常学习行走的过程，而且可能导致楼梯滚落等伤害。[6]第三种，是病理性的，比如严重的维生素D缺乏可引起佝偻病、进而导致腿型异常。O形腿的分类那么，哪些O形腿需要治疗呢？关键在于症状。如果7岁以后，O形腿的情况仍然没有缓解，或者2岁之后，O形腿的情况反而加重，再或者弯曲程度过重影响行走、身高明显低于平均水平等情况都应该考虑矫正。矫正方式有两种，一种是手术治疗，手术对人体的创伤比较大，因而有着严格的指证一般膝盖间距大于10cm才需要考虑。[1,3]矫正鞋垫，属于非手术治疗。大家可以试着模拟一下O形腿，拉开两膝距离后，脚也会随之调整，脚掌外侧受力增加、内侧则减少。使用具备支撑的鞋垫，可能存在两方面的益处。一方面，支撑可以改善脚底的受力情况，减少疼痛。国内外的研究，都证实了这一点。[7,8]另一方面，传统上认为，矫正鞋垫可以迫使小腿内旋、被动改变下肢发育，可以在一定程度上缓解症状。但是想达到彻底矫正的效果，目前尚没有办法下定论。[3]总之，不管是O形腿还是其他疾病，首先要厘清定义，最好向专业人士咨询；知道了症状才好了解发病机制，进而判断某种治疗方式是否恰当；最后，最好能找到国内外的研究数据，安全性如何、长期效果如何等等。符合这些标准便是好的，否则不管商家怎么吹嘘，都不要信。-----------------------------------参考文献[1]胡祖杰. 儿童X型腿、O型腿的评估与治疗进展[J]. 现代医药卫生, 2013, 29(10): 15121513.[2]YOO J H, CHOI I H, CHO T-J等. 医学类sci什么杂志好发

医学类SCI杂志期刊里没有绝对好发，或者说容易发的期刊。很多人会说低分的为灌水期刊，而高分的难发一些。不懂科研的人才会认为做科研存在最小工作量的颠覆性技巧。相对而言，OPEN ACCESS类的SCI期刊要比正常的容易一些，但是也并不代表说你什么都不下工夫就可以发表。优助医学认为，SCI发表的难易，应该以工作量来计算，最小工作量的需求来源于医生做科研时既经费局促又精力不足的窘境，基础条件很差的索性都去发META和临床研究的文章了。常规套路是细胞加上动物模型的实验，SCI，5分与1分的区别在于对临床资料各种分层分析的结果，以及获得有意义的结论。这一类研究，核心数据是三表一图：1）基线表；2）单因素表；3）多因素表；4）ROC曲线或者生存曲线图。

金锋的概况

金锋 博士，研究员，博士生导师 1993年毕业于东京大学理学部人类学系，获得博士学位。1993年回国后先后供职于中国科学院遗传研究所，1994年作为日本学术振兴会博士后研究员在京都国际日本文化研究中心研究亚洲人群血缘关系。1997年以来兼职于天津、内蒙古、西北等师范大学生命科学院客座教授，中国社会科学院考古研究所古代文明中心研究员，中央民族大学人类学学院客座教授，日本JBC生命科学研究所高级研究员，Head Scientist for New Sun Bio Science，Executable Inspector of Quality Assurance International (QAI USA）。 金锋博士研究涉及人类学、遗传学、分子医学以及共生微生物对生存质量的影响等多个领域。目前有3个主要在研方向： 1 成年人退行性病变的心理和生理基础 随着国家经济建设和生活质量的改善，人口平均年龄不断攀升，老年痴呆作为一种成年人退行性病变对迅速进入老年社会的中国影响严重凸显。心理压力、生活环境、饮食结构都有可能成为老年痴呆症的加速剂。老年痴呆需要一对一的护理，因此它是对国家经济发展负面影响最大的疾病之一。从分子水平和心理认知水平对老年痴呆症的早期表现的评估，可能为提前预知老年痴呆的发生提供重要依据，同时早期发现可以积极实施生物学干预，延缓发病时间或者发病进程。 2 人类及动物共生微生物的平衡对其行为的生物学影响 人类的身体可以负载着超过自身细胞数目100倍的共生菌(symbiosis bacteria），这些微生物大多生存在肠道中，并很大程度地影响着人类的生存质量。肠道是控制人类及某些哺乳动物情感的五羟色胺和多巴胺以及多种激素合成的重要场所，这些生化物质以各种传递方式通过血液或者肠道神经网络传递到人体不同位置，主动或被动产生一些可以感知的反应，因此肠道微生物群数量和比例与人类及动物行为有着十分密切的相关。很多有益微生物在肠道内占有较高比例时能够有效阻止硫化氢、氨以及吲哚类等神经毒物质在肠道内生成，降低人类及动物的应激和亢奋水平。硫化氢、氨以及吲哚类的降低对于减少糖尿病、肥胖症以及老年痴呆的发生又有着特殊的作用。研究微生物对基因表达调控和共生菌对人类及动物行为的影响；探讨抑郁症、自闭症以及多动症等心理疾患生物学纠正方法具有重要的科学意义。 3 气味偏好的心理学与生物学关联 研究表明，气味是动物群居乃至求偶的一个重要选择因素。人类是否有类似昆虫那样明显的外激素（pheromon）释放是人们长期关注的一个热点。即便是人类群体中在这方面也是有巨大族群差异（racial difference）。亚洲人群一个重要的群体特征就是位于16号染色体上的一个点突变造成了体味基因的缺失，致使大部分亚洲人不具有明显的体味。由此在世界上的族群通婚中亚洲人与欧美和黑人的通婚明显地少于其他族群之间的通婚。这种气味选择的生物学意义与人群的进化和分化之间的意义，心理学层面的影响以及生物学上的特殊选择，气味的偏好和人类及动物行为的关联是本课题关注的重要问题。人类外激素与体味的关联，外激素与青少年发育和心理状态的关联也是本课题关注的重要问题。 5年来发表的文章： 1.
